How do you motivate yourself to stay on a diet and exercise program?

I can stick to a diet and exercise regime for about five months but I can't seem to stay motivated over the long haul. I either get injured, or bored, or overconfident. Or something (like Covid) happens, and I fall off the wagon and undo all the good I have done. What are some tips to stay motivated over the long term?
